Microsoft Forms version of SORT

We've also made a Microsoft Forms version of SORT for ease of distribution when using it as a questionnaire. To use the forms, please follow the instructions below:

1. Pick a form suitable for your use-case, three versions are available for Nurses, Midwives and Allied Heath Professionals. Each version contains the same SORT questions but asks for different demographic information:

1. • SORT questionnaire for Nurses

   • SORT questionnaire for Midwives

   • SORT questionnaire for Nurses, Midwives and Allied Heath Professionals

2. Duplicate the form to your own Microsoft account using the "Duplicate it" button on the top of the screen as shown below:

 3.  A copy of the form will be saved to your own OneDrive storage.

 4. Modify the form as required, changing the title or adding/modifying demographic questions. We recommend that all SORT questions in category A to E are not modified.

 5.  When you're ready to collect responses, use the "Collect responses" button on the top right of the screen:

 6. You will be presented with various options for sharing the questionnaire, either by having Microsoft email participants directly or obtaining a link which you can manually share:
